I agree to fill out a seperate form related to the COVID-19 safety precautions

I confirm that the Salon will not be responsible or liable if the result of the service is not as expected.

I confirm that I will follow the regimen and the suggested follow-up care to maintain and treat my hair.

I am allowing the Salon to apply the necessary chemicals as part of the service in my hair treatment.

I understand that the result of this chemical may vary from one person to another.

I agree that the hairstyle is final after the service. If there are any changes requested after 1 hour of service completion, the client will be charged.

I provide consent for the Salon to take photographs of the provided service.

I provide consent for the Salon to share service photographs to social media for marketing campaigns or testimonial purposes.

I confirm that children are not allowed in the work service area for safety reasons.

I acknowledge that the Salon employees are licensed professionals and should be treated with respect at all times.

I acknowledge I have read this complete form and I accept the terms as indicated above.
